Code Of Conduct
As a client, it is your responsibility to:
Communicate your preferences, expectations and concerns.
Communicate complete and accurate health information and reasons for your visit.
Secure a signed Referral if under the supervision of a Medical Professional, prior to receiving any work.
Communicate with your Medical Professional of any work received/may receive and the results.
Treat staff and other guests with courtesy and respect.
Use products, equipment and therapies as directed.
Sign the Terms and Conditions stating you will Adhere to all the published policies and procedures.
As a client, you have the right to:
A clean, safe and comfortable environment.
Stop a treatment at any time, for any reason.
Be treated with consideration, dignity and respect.
Confidential treatment of your disclosed health information.
Trained staff who respectfully conduct treatments according to treatment protocols and the policies and procedures.
Ask questions about your service experience.
Information regarding training, licensing and certification.

To Better Serve You
Customer Satisfaction:
Your satisfaction is very important to us. To help ensure satisfaction clients are responsible for communicating any preferences, expectations, concerns, and adjustments that need to occur before and/or during the session. If you are not satisfied with your service please complete a 'Feed Back' card before leaving so we can know how better serve you in the future.
Hours of Operation/Rates:
Both Hours of Operation and pricing Rates are subject to change at anytime. The only notice of changes will be sent by e-mail to those whom have Opted-in to our e-mail list. Otherwise the must updated information can be found on our booking platform, or provided when requested.
Gratuity:
For your convenience, gratuity has been calculated into the base price for each service. No additional gratuity is required.
Add Ons:
For your convenience, all Add Ons have been calculated into the base price for each service. No additional fees are required.
Late Arrival for Appointment:
It is your responsibility to notify us if you are going to be late to your appointment. If you are late for an appointment, your treatment will end on time so that the next appointment may begin as scheduled. Should this happen, your service will be charged in full. If you choose to reschedule your appointment it will fall under the 24hr Cancellation/Reschedule policy. If you are more than 10min late and do not contact us regarding when you may arrive you will be considered a 'no show' and your appointment will fall under the No Show policy.
24 Hour Cancellation/Reschedule of Appointment:
We understand that unanticipated events happen in everyone’s life. In our desire to be effective and fair to all clients and therapists, the following policies are honored:
A credit card is required to be held on file to reserve all appointments, to be obtained prior to the first appointment.
We require at least 24 hour notice of cancellation or your service will be charged in full. If you notify us less than 24 hours, yet at least 2 hours prior to your scheduled appointment, that you need to reschedule the service will only be charged 50%. If you notify us of a need to reschedule and it is less than 2 hours before your scheduled appointment it will be considered a cancellation and charged as so. This policy is also honored in reverse if for any reason Limit Break Massage LLC requires the cancellation or rescheduling.
No Show:
In our desire to be effective and fair to all clients and therapists, the following policies are honored:
A credit card is required to be held on file to reserve all appointments, to be obtained prior to the first appointment.
If you do not show up for your appointment and no notice has been received with in 24hours of need to cancel or reschedule the service will be charged in full. If you are running late to your appointment it is your responsibility to notify us of your delay. If you do not notify us more than 10min after your scheduled time you will be considered a No Show where the service will be charged in full and no work will be administered even if you do arrive later. Any more than 2 No Shows will result in termination of your verified status and restrictions to making future appointments.

Events
To Better Serve You
Booking Your Event Request:
Contact us at with a full description of the requested Event Services, Date, Time and Location of the Event. Please note that there is a 2 hour minimum requirement.
Once booking request is receive, you will be contacted to discuss details and/or collect payment Information.
Event Pricing/Payment:
All prices are calculated based on time reserved, type of service, and distance of travel.
A single total may be calculated and provided if the Host is paying in full once all details are provided. A 2 hour pricing minimum is required regardless of event timeframe.
If Host request that clients/participants private pay for services a reservation and travel deposit will still be required of Host.
Event appointments are Not Confirmed until Payment is received, or a representative has contacted you to work out the details.
Parking and Stairs:
Please provide detailed parking and location instructions.
Reimburse the therapist if a parking fee is incurred.
Include flights of stairs & elevator access to assist therapist in estimation arrival time and preparing prober equipment.
Host gift:
For 'Private Pay Events', the Host is entitled to receive 1 complementary appointment for booking an event, 2 complementary appointment if booking 4hours or more. If the Host will not not be in attendance or will be unable to provide the information of the individual(s) receiving complementary appointments in person then the Host must provide the information prior to the event or will forfeit the Host gift.
Cancellation Policy:
We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients and therapists, the following policies are honored:
A credit card is required to be held on file to reserve appointment.
We require a 24 hour cancellation or reschedule notice or your event will be charged in full.
My Promise
Therapist cancellation/reschedule:
All Cancellation and Reschedule policies will be honored in reverse. If for what ever reason the therapist needs to cancel or reschedule within 24 hours of an appointment the client will not be charged and will be offered primary reschedule with no charge on rescheduled appointment.
Privacy Policy:
I respect the privacy of our clients. It is my policy:
To respect the purpose for which our site visitors/members gave us e-mail and contact information.
That Limit break Massage LLC will work with secure booking platform, and payment platforms, and will not sell, rent or loan customer/member lists to any other third parties for email, direct marketing or their purposes.
To send notices of upcoming appointments only, and will send special offers or other e-mail only to clients who opt-in.
To store personal information in a secure manner.
To not share/discuss any information regarding services received, personal conditions or resulting information found or discussed during sessions unless I receive written consent from client or under legal obligation.
To continue this policy beyond the termination of our company. If a majority portion of the assets of our company, which might include our subscriber and other lists, were to be acquired by another organization, it would be done only as part of a clear agreement binding that organization to abide by the spirit and letter of our privacy policy.

Massage Therapist neither diagnoses illness, disease, or any other medical, physical, or mental disorders; nor performs any spinal manipulations. The client is responsible for consulting a qualified physician for any physical ailment they may have.
